Workbooks

If you have been assigned a youth, parent or care management support, you have received corresponding workbooks in your welcome email. Paper copies will also arrive in the mail within four business days. No need to look through them yet, your interventionist will guide you along the way!

If you have not received them after one week, or if you need new copies please, text 206-420-5624 or email chip@seattlechildrens.org.

Gift cards

Gift cards are sent out after completing each round of surveys. Once ordered by research staff, you will receive an email with a redemption link. This can be redeemed at one of 17 different locations and does not expire.

You will receive an email from noreply@tangocard.com. Follow the instruction in the email to select the gift card of your choosing.
Gift cards are sent for completing surveys after signing up and at 6 weeks, 3 months, 6 months and 12 months.
